Name and complete address required to receive a fingerprint card. All incomplete submissions will be discarded. Fingerprint cards must be submitted with a completed S-100 form and an application.

Fingerprint cards are mailed out next business day after online submission. Please allow up to three weeks for mail delivery.

You may also obtain an FD-258 fingerprint card at most Law Enforcement Agencies.
